Refractive surgery: who can give up glasses and who cannot

Not everyone is a candidate, and not everyone needs the same technique. What really decides is the assessment beforehand.

«Having your eyes done» is not one operation: it is several different techniques, and choosing the right one is half the result.

The starting requirements

  • A stable prescription for at least a year. If it is still changing, you wait.
  • Being over 21, except in specific cases.
  • A healthy cornea of sufficient thickness: measured with topography and pachymetry.
  • Not being pregnant or breastfeeding, because the prescription fluctuates.
  • No significant dry eye and no untreated corneal or retinal disease.

Which technique for which eye

Laser

For moderate myopia, hyperopia and astigmatism with enough corneal tissue. It reshapes the cornea so that it focuses where it should. It is quick, done with drops, and sight recovers within hours or a few days.

Phakic lens (ICL)

For high prescriptions or thin corneas, where laser cannot be used. A lens is implanted inside the eye without touching the cornea and without removing the natural lens. It is reversible.

Multifocal intraocular lens

Past a certain age, when presbyopia has set in or a cataract is starting, the logical step is to replace the natural lens with one that corrects both distance and near vision. Two problems solved in a single operation.

The assessment is what matters

Before talking about surgery you have to measure: corneal topography and pachymetry, pupil diameter, prescription with and without dilation, intraocular pressure and the state of the retina. That assessment is what says whether it can be done, with which technique, and what result it is reasonable to expect.

If someone promises you a result before measuring your eye, be wary. The assessment first, the decision afterwards.
